Outcomes of vats anatomical resections in patients with primary lung carcinoma

ABSTRACT Background: Lung cancer is the most common cause of cancer-related deaths worldwide. It is the most common tumor in men and the third in women following breast and colorectal tumors. With advancing technology, lung cancer cases can be diagnosed at an earlier stage, increasing the popularity of minimally invasive procedures such as video assisted thoracoscopic surgery. Objective: Investigating the factors affecting the survival rate and prognosis in patients who had video-assisted thoracoscopic anatomic resections for primary lung cancer. Patients and methods: Patients who underwent video-assisted thoracoscopic surgery (VATS) anatomic resection between June 2006 and January 2018 were evaluated retrospectively through a prospective registry database of patients diagnosed with primary lung carcinoma in Istanbul Medical Faculty Thoracic Surgery Department. Clinical and pathologic stages of disease, cytologic diagnosis, type of anatomic lung resections, duration of drainage and hospital stay and postoperative complications were investigated. Two hundred and four patients were included to the study and patients who were not fully attained and whose data were incomplete were excluded. Results: The mean age of patients who underwent VATS anatomic resection for primary lung carcinoma was 69 ± 10. A total of 204 patients were included in the study, 141 of them were male and 63 were female. The mean FEV1 of the patients was 2440 ± 705 ml (90 ± 21%) and the mean DLCO values were 91 ± 24%. The mean hospital stay was 7.29 ± 5.6 days. The most common complications were pneumonia (14.2%) followed by atrial fibrillation (6.9%). The mean survival after resection was 104±4.5 months and 5 year survival rate was %85.6. Conclusion: In our study, it was determined that age, preoperative FEV 1 value, duration of hospital stay, postoperative complication development, histopathologic diagnosis of the tumor and stage of the disease were statistically significant in the survival of patients who underwent VATS anatomic resection for primary lung cancer. It was concluded that intraoperative conversion from VATS to open thoracotomy and the difference between segmentectomy - lobectomy did not have a specific effect on survival.
