Diagnostic value of ischemic modified albumin in acute abdomen

Abstract (EN)

Objectives: Patients with acute abdominal pain are one of the most frequent patient group which are seen in emergency department. Differential diagnosis, determination of surgical treatment or not, whether discharge or not of patients like this make big difficulties for emergency doctors. These difficulties encourage to do studies to find appropriate biochemical markers for initial diagnosis.Methods: 296 objectives included to the study. Control group was created with 130 objectives and patient group with 166. The value of IMA compared within 166 patient in patient group and 130 healthy person in control group. Relationship between the value of amylase, D-Dimer, White Blood Cells (WBC), clinical findings of objectives in patient group and IMA levels of these patients, and diagnostic value of IMA in acute abdominal pain investigated.Findings: Regarding to the diagnostic classification of patients who included in the study the biggest group was made by 51 (30.7%) patients with non-specific abdominal pain. Ultrasonography (USG) performed in 118 (71.1%) of 166 patients. The gall bladder pathology found in 33 (28.0%) individuals of this patient group. Computerized tomography (CT) performed in 37 (22.3%) patients and apandisit found in 4 (10.8%), and mesentery ischemia found in 6 (16.2%) of them. On the other hand there was no pathology in 12 (32.4%) patients. The mean leukocyte value of 166 individuals who included to the study was 11.3590 ± 5.29084 mm3, mean value of D-Dimer was 3.0955 ± 5.88847 µg/ml and mean value of amylase was 215.67 ± 519.889 u/L. mean value of IMA in patients with acute abdominal pain was 0.50737 ± 0.204 ABSU ( P< .05 ) and this value in the healthy control group was 0.56883 ± 0.185 ABSU (P< .05 ) and due to this findings the mean value of IMA in patient group was significantly lower than mean value of IMA in control group.Conclusion: The serum IMA values in patients with acute abdominal pain is not useful as a diagnostic parameter. We believe that multi central and wide serial studies about this topic will give more healthy and exact results.
