The retrospective study of acute respiratory distress syndrome patients in term of pulmonary and extrapulmonary causes

Abstract (EN)

Objektives: In this study, ıt is intendted to identify the ARDS incidence, etiologic factors, mechanic ventilation, multiorgan disfunction and mortality ratios of anesthesiology and reanimation intensive care unit.Equipments and procedures: In this study 121 ARDS patients has been examined. The patents are separated in two ARDS patients groups; pulmonary (n:70) and extrapulmonary (n:51). All of the patients has been exposed to volume controled SIMV mode mechanic ventilation. These patient?s demographic characteristics, etiologic factors, arteriol blood gas volues, mechanic ventilation parameters, GKS, APACHE II and MPM II scores, hypoxia index (Hİ), Murray index, oxygenation index (Oİ), alveoloarterial oxygen gradient (AaDO2), multiorgan disfuntion scores (MODS) and mortality ratios has been recorded.Findings: In anesthesiology and reanimation intensive care unit, the ARDS incidence of %7 and mortality ratios of %48 has been obtained. Underlying predisposing factors are, travma, aspiration pneumonia, postoperative respiratory failure and pneumonia. %57 (n:70) of the patients has pulmonary ARDS and %42 (n:51) of the paitents has extrapulmonary ARDS. Keeping track of both groups daily, no noticable difference has been observed in the mean arterial blood gas valves, Hİ ratios, Oİ ratios, Murray index valves and AaDO2 valves. In addition to that both of the groups has similar GKS, APACHE II, MPM II and MODS ratios. In pulmonary ARDS group, %47 of 70 patients has been mortal while in extrapulmonary ARDS group, %50 of 51 patients has been mortal.Conclusion: In this retrospective study, we have observed similar and positive results in pulmonary and extrapulmonary ARDS group with acceptable lung protevtive mechanic ventilation. Even though the extrapulmonary ARDS has been observed less frequent than the pulmonary ARDS, the mortality has been recorded to be higher. It has been observed that the extrapulmonary and the pulmonary ARDS groups has been responded similarly to the treatments. It is conculuded that early diagnosis and proper treatments has been effective on mortality.Key words: ARDS, lung protective mechanic ventilation, mortality.
