Clinical results of the patients of rotator cuff rupture repaired with all suture anchor material and the investigation of the causes of pulled out anchor

Shoulder joint is important for the sportive and lifetime activities. Rotator cuff muscles are the major functional structures of the shoulder joint. Surgical and conservative treatment methods are used in rotator cuff tear. To improve the success rate of rotator cuff tear repair many of surgical techniques and materials were developed. The aim of this study is to reveal the clinical results of the patients with rotator cuff tear and repaired with all suture anchor material, comparise the results with the other anchor materials in literature and investigate the reasons of the pullout situation. In our clinic, 131 patients were diagnosed rotator cuff tear with clinical and radiological methods repaired with all suture anchor arthroscopic single row technique between January 2015 to December 2018. 79 patients were female(60.3%), 52 patients were male(39.7%) and the average age was calculated as 60.78(35 to 80). Mean follow up time was 26.63 months(14 to 54 months) Evaluation of the preoperative and postoperative Constant-Murley scores, UCLA scores and VAS scores reveals statistically significant improvement(p<0.001, p<0.001, p<0.001). Considering the Constant-Murley scores in our study, it was found that the results of all-suture anchors in our study were similar when compared with the results of rotator cuff repair studies performed with metal anchors in the literature. Mean age of the patients had pullout was 68.71 and that was found significantly higher than the general patient population(59.83). 14 patients who had pullout(3 of them were osteoporotic) were calculated with an average BMD(bone mineral density) of -2,04. Also 2 patients who had pullout were diagnosed with cyst in humerus head. As a final conclusion increased risk of pullout was detected in patients with older age, lower BMD values and humeral head cysts. Except those situations all suture anchor material was shown as safe and effective option according to its satisfactory clinical results.
