Evaluation of neuropathic pain frequency in patients with ankylosing spondylitis

Evaluation of Neuropathic Pain Frequency in Patients with Ankylosing Spondylitis Objectives: In this study, it is aimed to determine the prevalence of neuropathic pain in patients with ankylosing spondylitis who applied to our clinic, and to retrospectively examine the relationship of neuropathic pain frequency with disease activity, quality of life, clinical, demographic, laboratory data and current medical treatment. Material and Method: A total of 136 patients, male and female, aged 18-73, who applied to the outpatient clinic of Karadeniz Technical University Faculty of Medicine, Department of Physical Medicine and Rehabilitation, fulfilling ASAS classification criteria for radiographic axial SpA (Ankylosing Spondylitis), and had been receiving the same treatment for at least 3 months, were included. The presence of neuropathic pain was evaluated according to the LANSS and DN4 scales. A comparison was made between the group with and without neuropathic pain in terms of sociodemographic characteristics, disease duration and activity, quality of life, clinical and laboratory data, and current medical treatment. Results: Neuropathic pain was detected in 33 (24.3%) patients according to LANNS and 36 (26.5%) according to DN4. In patients with neuropathic pain, the rate of female patients, low education level, and unemployed patients was higher. Patients with neuropathic pain had higher disease activity levels, and poor functional status and quality of life. According to LANSS, it was observed that the rate of use of anti-TNF agents in patients with neuropathic pain was significantly lower. It was determined that the mean values of BASDAI, BASFI, Patient global-VAS, Doctor global-VAS, ASDAS-CRP, ASDAS-ESR, ESR and ASAS HI were lower in male patients than in female patients. Conclusion: In conclusion, it should be considered that pain has a neuropathic component in AS patients. Neuropathic pain component has been shown to be associated with high disease activity and poor functional status. As a result, a decrease in the quality of life of the patients is observed. In the treatment, v neuropathic pain should be considered along with inflammatory pain. Neuropathic pain assessment should also be included in patient assessment scales and unnecessary drug switches should be avoided.
