The correlation between abnormal cervical cytology results and human papilloma virus infections

Our objective is to evaluate the correlations between abnormal cervical cytologyresults according to the Bethesda 2001 terminology and colposcopic findings,histopathologic results, HPV-DNA seropositivity, biopsy procedures and age.Number of 139 patients who were admitted to Gazi University Faculty ofMedicine, Department of Obstetrics and Gynecology from 2006 to 2007 were included toour study. A clinical descriptive study was designed. Study group included 71 ASCUS, 35LSIL, 14 HSIL, 12 AGC and 7 ASC-H cases. We have evaluated associations betweencytologic groups and histopathologic, colposcopic findings, HPV-DNA seropositivity,biopsy procedures and age.By the results of our study, a significant correlation was not found between the agegroups and cytologic, histopathologic results, colposcopic findings, HPV-DNAseropositivity and biopsy procedures.A significant correlation was found between cytologic results and HPV-DNAseropositivity, biopsy procedures and histopathologic results, but was not found withcolposcopic findings.A significant correlation was found between the colposcopic findings and biopsyprocedures, histopathologic results, but was not found with HPV-DNA seropositivity.83A significant correlation was found between the biopsy procedures andhistopathologic results, but was not found with HPV-DNA seropositivity.A significant correlation was found between HPV-DNA seropositivity andhistopathologic results.For all cytologic groups HPV-DNA seropositivity was detected at low rates and alow sensitivity value was calculated for predicting CIN2+3.When we evaluated the association between cytologic and histopathologic findings,we have the comment of there has been an overdiagnosis of cytologic results and highgrade lesions were at low rate in the histopathologic results.We have observed that there is a 50% dicrepancy between the colposcopic findingsand histopathologic results.It has been seen that there is an overdiagnosis and overtreatment by using leepprocedure for ASCUS and LSIL cases.By the result, we have suggested that using a combination of HPV testing andcytology is not suitable for the triage of cervical cancer screening, but HPV-DNA test canbe used for the posttreatment management of histologically confirmed high grade cervicalintraepithelial lesions.
