Abstract (EN)
Investigation of benign or malign endometrial pathology risk factors in patients presenting with abnormal uterine bleeding, Dr. Elif Mutlu OĞUR. Abnormal uterine bleeding (AUB) is one of the most common clinical conditions requiring gynecological evaluation all over the world, impairs the health and well being of women, and has negative effects on their quality of life. Although the majority of AUBs are not associated with a premalignant or malignant lesion, it has been reported that especially AUBs in the postmenopausal period, as well as premenopausal AUBs containing additional risk factors, may be associated with endometrial cancer. This study aims to compare the endometrial biopsy results of women admitting with AUB, to determine the risk factors associated with benign or malign endometrial pathologies. This cross-sectional study using the retrospective record scanning method was conducted with 100 women over 18 years of age who applied to the gynecology and obstetrics clinic with AUB and underwent endometrial biopsy. Age, body mass index (BMI), obstetric and gynecological history, medical history, and pathology results of the patients were recorded. Endometrial pathology results were classified as normal, benign, and malignant. The effects of the demographics and clinical features of the patients on the risk of developing benign and malignant pathology were investigated. The mean age of the participants was 48.7±7.7, and the mean BMI was 29.3±5.9 kg/m2. Of the patients, 59 (59.0%) are in the premenopausal period, and 41 (41.0%) are in the postmenopausal period. While the ultrasonographic endometrial thickness of 23 patients is below 8 mm, 27 of them are between 8-11 mm and 50 are 12 mm and above. The pathology result was found to be normal in 35 (35%) of the patients, benign pathology was found in 45 (45.0%), and malignant pathology in 20 (20.0%). In multivariate analyzes, every 1-year increase in the age of the patients increases the risk of developing malignant endometrial pathology by 1.17 times, and each 1 mm increase in endometrial thickness increases the risk of developing malignant endometrial pathology 1.16 times. The cut-off point for ultrasonographic endometrial thickness was found to be>12 mm. According to this cut-off point, the sensitivity of ultrasonographic endometrial thickness in estimating endometrial pathology was 70% and the specificity was 62.9%. Identifying malignant disease risk factors in women presenting with AUB and early application of invasive methods such as endometrial biopsy in women with risk factors may directly affect the success of treatment
