Is appendectomy sufficient for appendiceal tumors?

Title of the Reseach: Is appendectomy sufficient for appendiceal tumors? Objective: Primary tumors of the appendix are extremely rare. They are mostly detected incidentally after appendectomy that was performed for any reason. It has been an ongoing debate for many years whether additional surgical intervention is required after appendectomy. In this study, it is aimed to investigate whether appendectomy performed in patients diagnosed with appendix tumor is sufficient. Patients and Methods: Patients undergoing appendectomy for any reason between October 2006 and October 2019 were retrospectiively reviewed. Patients with primary tumoral lesions in the appendix were included in the study. The patients were evaluated in terms of biological behavior groups, histopathological types, preoperative diagnosis, the first surgery and whether to have a second surgery. Results: It was determined that there were 132 patients [Male / Female: 45 (34,1%)/ 87(65,9%)] diagnosed with appendix tumor. There were 27 (20,5%) patients in "benign" category, 61 (46,2%) patients in "borderline" category, and 44 (33,3%) patients in "malignant" category. While 105 (79,5%) of the patients underwent "simple appendectomy" in the first operation, 27 (20,5%) patients underwent "extensive resection" . It was observed that 18 (17,1%) of 105 patients who underwent appendectomy in the first operation were applied wide resection, and 87 (82,9%) patients did not undergo additional surgery. Conclusions: It was concluded that appendectomy is sufficient in benign tumors and early- stage borderline tumors. In early-stage malignant tumors, there is a histopathological subtype (neuroendocrine tumor) in which appendectomy is adequate. In advanced tumors, aggressive surgical procedures should be applied in experienced centers because they provide survival benefit.
