Evaluation of adiponectin, leptin levels and oxidative stress in bipolar patients with metabolic syndrome which treated vaiproic acid

PURPOSE: An increased risk for metabolic syndrome (MS) has been described for persons with psychotic and mood disorders. The aim of the study was to determine the influence of valproic acid (VPA) treatment on adiponectin, leptin levels and oxidative damage in bipolar disease.MATERIAL AND METHOD: 40 bipolar patients receiving VPA mono therapy were included in this study, with 20 healthy control subjects. Patients are divided into two groups with and without metabolic syndrome. 20 bipolar patients, diagnosed according to the DSM IV, were assessed for metabolic syndrom according to the NCEP ATP III criteria. Adiponectin, leptin, protein carbonyls, -SH groups and MDA levels were measured to evaluate in 40 bipolar patients and 20 control subjects.FINDINGS: Serum adiponectin levels were significantly lower in group 1 patients than group 2 and control subjects (p<0,001). Serum leptin levels were significantly higher in group 1 patients than group 2 and control subjects (p<0,001). Serum ?SH levels were significantly lower in group 2 patients than group1 (p<0,001) and control subjects (p<0,05). Serum carbonyl levels were significantly higher in group 1 and group 2 patients than control subjects (p<0,001). Serum MDA levels were significantly higher in group 1 patients than group 2 and control subjects (p<0,001).CONCLUSION: Recently, long-term antiepileptic treatment with valproate increases the risk of metabolic disorders such as insulin resistance and/or obesity. However Genetic factors, sedantery life style, inappropriate diet habits, smoking and progressive weight gain play role in developing MS. We concluded that neverthless of pharmakologic agents, healthy life standarts like physical activity and ideal body weight are usefull for the treatment.
