Abstract (EN)

Bruxism; It is a parafunctional movement in the form of clenching and grinding, which affects a significant part of both the child and adult population. Bruxism can be primary (idiopathic) or secondary. Primary bruxism is divided into two subgroups, sleep and awake bruxism, which are considered clinically irrelevant. Sleep bruxism (SB) is involuntary and is classified as a sleep-related movement disorder. Awake bruxism (AB), by contrast, is defined as awareness of clenching. Secondary bruxism is; It has been observed as a side effect of drug use and in some neurological and developmental disorders. The aim of our study is to examine the stress changes that will occur in the temporomandibular joint disc as a result of botox application to the masseter and temporal muscles in bruxism patients with 3D finite element analysis (FEA). In total, three different skull models were created: 1-the model of the bruxism patient, 2-the model in which only botox was applied to the masseter muscle, and 3-the model in which botox was applied to the masseter and temporal muscles. In Botox application, masseter muscle and temporal muscle strengths were reduced by 40%. Von Mises stress distributions in the masticatory muscles, articular fossa of the TMJ, mandibular condyle and joint disc during clenching of the teeth in all three models were examined. When the results of the study were evaluated, the stresses in the disc and fossa were reduced by 25%. For this reason, it can be recommended to apply temporal and masseter botox together in the prevention of advanced disc diseases.
