Postoperative complications after living donor hepatectomi: First 500 case

Background: This study retrospectively analyzed the experience of the donorscomplications in living donor liver transplantation depending on the Clavienclassification system to define, and graded the severity of these complications in 500cases.Materials and methods: We retrospectively identified and analyzed thecomplications of 500 alive consecutive donors underwent RH or LH for LDLT betweenSeptember 2005-August 2011.Results: Totally 152 complications was observed in 96 of the 500 (19.2%)donors. The cases followed up means 30 months. There was no donor mortality.Complications developed into 88 (19.5%) RH donors, 5 (35.7%) LH donors and 3(8.5%) LLS donors. The incidence of reoperation was 5.4% in all donors. Seventy-sixof 152 complications were grade I (50%), and 9 complications were grade II (6%).Major complications consisted of 28 grade IIIa (18.4%), 35 grade IIIb (23.4%) and 4Grade IVa (2.6%). Grade IVb and grade V complications did not occur. The mostcommoncomplication was biliary complications, which seen in 54 of 152complications (35.5%). As a result donor surgery for LDLT has a lot of complications.Conclusions: Experienced centers, reason the patient selection andintensive postoperative follow-up of healthy subjects controllable low morbidity can beachieved safely in liver grafts.
