The effect of perioperative care differences on postoperative outcomes in dentofacial deformity surgeries (maxilla and/or mandible)

Abstract (EN)
Objective: The aims of this study are preventing pain and postoperative nausea vomiting (PONV), shortening postoperative care unit (PACU) and hospital discharge times, providing satisfaction in patients undergoing dentofacial deformity surgeries by the use of Enhanced Recovery After Surgery (ERAS) protocols. Material and methods: After Ethics Committee approval, data of 60 patients who underwent elective bimaxillary dentofacial deformity surgeries, were investigated retrospectively. Following standard monitorization and general anesthesia; Group 1 received conventional approach and intraoperative 10 mL/kg/h IV izolen infusion, when Group 2 received ERAS protocol. Group 2 did not preoperatively smoke for 48 hours and drank clear liquids until the last 2 hours, the patients received 6 mL/kg/h IV izolen infusion intraoperatively. In these; gastric aspiration was applied before extubation, and for the first postoperative 48 hours PONV prophylaxis was supported, patient controlled analgesia was added to the analgesia plan. Numeric rating scales (NRS), analgesic consumptions, PONV incidences, PACU and hospital discharge times, satisfaction scores of two groups through the postoperative first 48 hours were compared. Results: During postoperative first 48 hours; in Group 2 NRS scores were lower, especially on 2nd, 24th and 48th hours (p<0.05). The number of patients described PONV were also lower, significant on 0th min (p=0.031). In Group 2, PACU discharge time was shorter (p=0.038). Total analgesic consumptions and hospital discharge times were similar in both groups, patients were more satisfied in Group 2 (p<0.05). Conclusion: We believe that the comfort and satisfaction of patients undergoing dentofacial deformity surgeries can be improved by ERAS protocols.
