Doğanay silikon yaği çikarimi sistemi ve yöntemi

SUMMARYPURPOSE: To investigate the effectiveness of a new system and method which are named `?Doğanay silicone extraction system and method?? for 1000 cSt and 5000 cSt silicone oil removal.MATERIAL AND METHODS: 149 eyes of 149 patients which are removed 1000 cSt (Group 1; 133 cases) and 5000 cSt (Group 2; 16 cases) silicone oil between February 2010-March 2012 in our vitreoretinal surgery department were enrolled. All cases? indications for PPV, duration between PPV and silicone oil removal, silicone oil removal time, additional surgery procedures, number of sclerotomy/the necessity of sutur for 23 G sclerotomy sites, intraoperative and postoperative complications, best corrected visual acuity (BCVA) changes (LogMar), and intraocular pressure changes were evaluated.RESULTS: BCVA at before PPV surgery, silicone oil removal and last visit were [2,16±0,97, 1,39±0,74, 1,23±0,88], [2,17±0,77, 1,66±0,83, 1,40±1,05], respectively in group 1 and group 2. During surgery, only one complication was occured in group 1. During surgery, no complication was seen in group 2. During the follow-up period, different complications in 13 cases were seen in group 1. Only one complication was seen in group 2. Mean follow-up period were 7,2±6,2, 4,6±3,2 (months), respectively in groups. The mean time between PPV with silicone oil endotamponade and silicone oil removal were 8,5±3,5, 10,7±6,2 months, respectively in groups. Mean silicone oil removal surgical time were 141,7±37,7 seconds, 426,6±102,2 seconds, respectively in groups. 293 of total 210 and 37 of total 26 sclerotomies sites required suture, respectively in groups. Lens status was phakic/pseudophakic/aphakic (57/69/7, 8/8/-) in groups. A total of 133 patients, 40 patiens in group 1 were receiving glaucoma treatment. Nine cases were above 21 mmHg despite treatment in group 1. Two of these patients, IOP were controlled with Mitomycin-C trabeculectomy surgery. Two of these patients, IOP were controlled with Ahmed Glaucoma Valve surgery. Ocular hypotony occured in 23 cases the day after surgery, in group1. A total of 16 patients, 2 patients in group 2 were receiving glaucoma treatment. Ocular hypotony was observed only one patient the day after surgery, in group 2. Some patients in group 1 and group 2, different anterior and posterior segment surgery procedures were applied during silicone oil removal.CONCLUSIONS: Removal of silicone oil of 1000 cSt with `?Doganay method and system?? are fast, safe, easy, economic, and effective.
