Evaluation of the effect of effective diabetes therapy on cognitive functions, BNP and hSCRP parameters

Abstract (EN)
Diabetes is a disease that affects millions of people of all ages, genders and races and has a high incidence. Chronic hyperglycemia due to diabetes is known to be the cause of microvascular and macrovascular complications in the long term. Among these, cardiovascular system and central nervous system complications have an important place. It is known that the severity of cardiovascular diseases increases even more in the presence of diabetes. It is still not known exactly how diabetes affects cognitive functions. For this reason, the effect of the presence of diabetes on cognitive functions is currently among the important research topics. In our study, it was aimed to evaluate the effects of diabetes treatment on cognitive functions, BNP and hs-CRP parameters in patients with Type-2 diabetes. Diabetes treatment was divided into two groups according to HbA1c levels as controlled diabetes (HbA1c ≤ 7) and uncontrolled diabetes (HbA1c > 7). A total of 18 patients were included in the study. BNP and hsCRP levels were evaluated by ELISA method. The MMSE test was used to evaluate cognitive functions. The routine laboratory parameters (if any) of the participants in the last month were evaluated from their electronic health records. A statistically significant difference was found between the groups with controlled and uncontrolled diabetes in terms of blood glucose level and HbA1c level (p<0.05). The mean MMSE score of patients with uncontrolled diabetes was lower than the mean of MMSE score of patients with controlled diabetes, but the difference was not statistically significant (24.86±4.38 and 26.64±2.46, respectively, p=0.425). No statistically significant difference was found between the groups in terms of (BMI) and HbA1c (p>0.05). Although higher BNP and HsCRP levels were detected in the uncontrolled patient group compared to the controlled patient group, the difference did not reach the level of statistical significance (BNP levels for controlled and uncontrolled diabetes groups, respectively: 168.51±71.15 and 156.71±69.93 pg/ml, p=0.860; HsCRP levels: 5.07±3.75 and 4, 00±2.96 mg/L, p=0.509). Clinical studies in larger populations are needed to evaluate in detail whether effective treatment in patients with type 2 diabetes has a potential effect on cognitive dysfunction and neurodegenerative diseases such as dementia.
