COMPARASION OF MAGNETİC RESONANCE CHOLANGIOPANCREATOGRAPHY AND ENDOSCOPIC RETROGRADE CHOLANGIOPANCREATOGRAPHY IN PATIENTS WITH EXTRAHEPATIC CHOLESTASIS FINDINGSPURPOSECholestasis is a situation that occurs within the blockage of the bile in the bile ducts inside the liver and the common bile duct. Biliary obstruction could be formed by some reasons such as choledocholithiasis, tumors, complications after gallbladder surgery and trauma. Choledocholithiasis is the most common reason.Some authorities claim that the new, fast, high-capacity MRCP sequences are better than ERCP. The aim of this study is to compare the high T2-weighted MRCP and ERCP diagnostically to define the pancreatobiliary diseases.MATERIAL METHODIn this study, the cases that resorted to Adnan Menderes University Hospital, Department of Gastroentorology between February 2007 and May 2010 who had suspicion of extrahepatic biliary disease with clinical and laboratory findings were taken to the study.The biochemical parameters, hematological tests of the patients that were taken into the study were reached from their patient file information. The cases that had suspicion of extrahepatic biliary disease were compared with their performed MRCP and ERCP results. The specifity and sensitivity of the MRCP was evaluated. Two methods were compared within the findings of the ERCP. In the evaluation of data, SPSS 14.0 Windows programme was used.FINDINGSThe mean age of the patients that were taken into the study was 58.76±17.85. The reason of the cholestasis of our 21 cases that had MRCP and ERCP with the diagnose of extrahepatic cholestasis was reported as choledocholithiasis 9 of them due to the MRCP findings and 14 due to the ERCP findings. In our cases, MRCP?s sensitivity was 57.1% for choledocholithiasis and 66.7% for malignancy.CONCLUSION:ERCP is considered as gold Standard in extrahepatic cholestasis. In our study MRCP and ERCP compared in biliary obstruction diagnosis and correlation results were as follow: 100% in specifity, 66.6% in sensitivity, 94.7% in positive predictive value, 100% in negative predictive value. There was no statistically difference between two methods in diagnosing (P=1.00). Thus we think that within the introduction of new imaging techniques such as MRCP, ERCP may be useful as a therapeutic process.Key Words :MRCP, ERCP, Extrahepatic Cholestasis
