Abstract (EN)

Aim: It was aimed to evaluate the demographic characteristics of patients diagnosed with acromegaly, laboratory values at the time of diagnosis and follow-up, clinical symptoms, comorbid conditions during the follow-up, treatment methods applied and response rates after treatment. Material and Methods: 71 patients with acromegaly who applied to Karadeniz Technical University Faculty of Medicine Endocrinology and Metabolic Diseases polyclinic between 2010 and 2020 were included in the study. Demographic characteristics of the patients, laboratory values and clinical features at the time of diagnosis and follow-up, and selected treatment methods were reviewed retrospectively. Results: %45,1 of the patients were female and %54,9 were male. The mean age of the cases at the time of diagnosis was 48,5 years. The most common complaint of the patients was enlargement of the hands and feet (%53,5). At the time of diagnosis, %35,2 of the patients had hypertension, %23,9 had diabetes mellitus, %1,4 had cardiovascular disease, and %5,6 had a history of malignancy. At the time of diagnosis, %2,8 of the patients had secondary adrenal insufficiency, %1,4 had secondary hypothyroidism, %7 had hypogonadism, and %2,8 had panhypopituitarism. Pituitary acromegaly was diagnosed in %97,2 of the patients and non-pituitary acromegaly was diagnosed in %2,8. Of the patients with pituitary adenoma, %71 were macroadenomas and %29 were microadenomas. The mean adenoma size was 15,3±7,8 mm. The primary treatment regimen chosen in %76,1 of the patients was surgery. %40,8 of the patients were surgery treatment only, %40,8 surgery + medical treatment, %8,5 medical treatment only, %5,6 surgery + medical treatment + radiotherapy, %2,8 medical treatment + radiotherapy , %1,4 received chemotherapy. It was observed that %49,3 of the patients were in remission after all treatment regimens. In the follow-ups of the patients, it was found that %4,2 of the patients developed colonic malignancies and %5,6 of them developed thyroid malignancies. In the follow-up of the patients, low EF was observed in %4,2, and medium-advanced valve insufficiency developed in %8,4. During their follow-up, hepatomegaly was found in %33,8, splenomegaly in %1,4, and hepatosplenomegaly in %4,2. Conclusion: In our study, the characteristics of patients with acromegaly were generally found to be similar to the literature. Early diagnosis of the disease is important in terms of treatment and response to treatment, and reduction of comorbidities.
