Abstract (EN)

In fertile and infertile cases the presence of seminal plasma chlamydiatrachomatis and human papilloma virus infection with the effect seminal fluidcytokines and sperm parametersTamer ÇALIŞKANII. ABSTRACTInfertility affects approximately 10-15% of couples. Roughly 60% of casesinvolve a male contribution or factor. Genital infections affect fertility by destroyingspermatogenesis and sperm function or by obstructing seminal tract. HPV andChlamydia trachomatis are two of the aetiologic agents of genital infections. Theyalso have an influence on infertility.In our study, we aimed to determine the prevalence of HPV and Chlamydiatrachomatis positivity in patients that admitted to Urology polyclinic of AdnanMenderes University Hospital. And, we also aimed to investigate the effects of thesemicroorganisms on semen parameters and the cytokine levels of seminal fluids.All of the cases who admitted to our Urology polyclinic for semen analysis in2006 were evaluated. 144 of the infertile cases and 31 fertile cases were included inour study. Chlamydia trachomatis and HPV positivity were investigated by usingNucleospin Tissue Macharey-Nagel extraction commercial kit with PCR and IL-18level were measured using ELISA (Biosource cat:KHC0181) method in seminalfluid.The prevalence of HPV positivity was determined as 1.1% (n=2), Theprevalence of Chlamydia trachomatis was determined as 8.6% (n=15). Howeverthere was no significant difference in HPV positivity between fertile (%3.2, n=1) andthe infertile group (%0.7, n=1) (p> 0.05). Likewise the difference of Chylamydia3trachomatis positivity between fertile group (%9.7, n=3) and infertile group (%8.3,n=12) wasn?t significant (p> 0.05).When we evaluated the prevalence of Chlamydia trachomatis and HPVpozitivity in subgroups of varicocele, oligospermia, azospermia, asthenozoospermia(occurance), there were no statistically significant differences between subgroups (p>0.05).There were measurable levels of IL-18 in all cases. These levels werebetweeen 2,3 pg/ml ile 234,3 pg/ml (mean value 40,25± 38,18 pg/ml). The meandifference of IL-8 levels weren?t significant between fertile (43,73± 37,81) andinfertile (39,50± 38,38) group (p>0.05). And when we compared the IL-18 levels andthe mean value of sperm number and motility, there were no statistically differencebetween the patient of Chlamydia trachomatis positivity and control groups.As a result, The prevalence of HPV and Chlamydia trachomatis positivity inUrology out patient clinic were determined by this first study that was made in ourcountry. There were no statistically differences between the presence of Chlamydiatrachomatis and sperm parameters. So, we think that incidentally determinedChlamydia trachomatis in semen doesn?t indicate as cause of infertility.Finally, this study also showed us that IL-18 level isn?t a marker ofasymptomatic genital infections. However, further large studies are needed to revealthe effects of HPV on semen parameters in infertile cases.4
