The relationship of prenatal care satisfaction with normal brith tendency in pregnant women

Abstract (EN)

This study was conducted to determine the relationship between prenatal care satisfaction and normal birth tendency in pregnant women. The descriptive and correlational study was conducted with 404 pregnant women with a healthy singleton pregnancy of 28 weeks or more who applied to Tokat State Hospital, Erbaa State Hospital and Tokat Gaziosmanpaşa University Hospital for follow-up purposes between January 30, 2023 and April 2023. In the study, "Pregnancy Identification Form", "Prenatal Care Satisfaction Scale (PCSS)" and "The Belief Scale for Normal Delivery (BSND)" were used as data collection tools. The data were analyzed with the statistical package program (IMB SPSS 22.0). When the averages of the participants were analyzed; age was 27.12±5.37, age at marriage was 22.50±3.78, gestational week was 33.80±3.99, and weight gained during pregnancy was 10.58±1.27. The most of the participants were women who had high school education or less, were housewives, had an income equal to their expenses, had no previous childbirth experience, conceived for the first time, conceived voluntarily, learned about their pregnancies by taking a pregnancy test, and had received at least one PC service. The mean total score of the women was 71.40±13.12 and the mean subscale mean scores were as follows: art of care 22.72±4.30, technical quality 13.44±2.69, physical.70±2.71, accessibility 12.51±2.45 and convenience 10.01±2.17. The mean total score of the BSND was 77.10±11.14, and the scale subscale mean scores were as follows: perceived sensitivity 9.52±2.79, perceived seriousness 14.38±3.65, perceived benefit 14.44±3.16, perceived barriers 13.80±5.37, perceived self-efficacy 14.09±3.52 and perceived health motivation 10.84±2.70. There was no significant difference between the total and sub-dimension mean scores of the PCSS according to the sociodemographic and obstetric characteristics of the participants (p>0.05). There is no significant difference between the mean total and subscale scores of the BSND according to the sociodemographic characteristics of the participants (p>0.05). There was no significant difference between the mean scores of the participants according to their obstetric characteristics (p>0.05). However, the barriers perceived by women who learned about their pregnancy by looking at the pregnancy symptoms by themselves were significantly higher than the barriers perceived by women who learned about their pregnancy by visiting a doctor. In this study, the barriers to normal delivery perceived by women who received PC services from private hospitals were higher than those who received PC services from FHCs. Again, the self-efficacy perceptions of women who received the PC service from FHC were higher than those who received the PC service from public and private hospitals. In the study, a low-level positive correlation was found between the total scores of the PCSS and the total scores of the BSND (r=0.216, p<0.05). A low-level negative correlation was determined between the total scores of the BSND and gestational week (r=0.099) (p<0.05). In this study, for satisfaction in prenatal care, as the art of care increases, the perceived seriousness and benefit of normal delivery increases. As the technical quality and appropriate care for satisfaction in prenatal care increases, the perceived seriousness, benefit and health motivation for normal delivery increases. As the technical quality for satisfaction in prenatal care increases, the perceived barriers to normal delivery decreases. In this study, as the physical environmental conditions for satisfaction in prenatal care increased, the perceived sensitivity and seriousness of normal delivery increased. For satisfaction in prenatal care, as women's level of accessibility to health care increased, perceived sensitivity, seriousness and health motivation for normal delivery increased. It was found that the variables of the total score of the PCSS and gestational week were significant predictors of the total scores of the BSND (p<0.05). According to this model, the variables of the total score of the PCSS and gestational week explained 22.7% of the BSND scores. As the satisfaction of pregnant women with the care they received in the prenatal period increased, their level of belief in normal delivery increased. As the gestational week progresses, the level of belief in normal delivery decreases. To increase women's tendency towards normal delivery, it is important to increase efficiency and satisfaction in prenatal care and to demonstrate an individualized supportive approach.
