Gestasyonel diyabetli hastalarin ve sağlikli gebelerin periodontal mikrobiyolojik profilleri

The aim of this study is to evaluate the association between gestational diabetes mellitus (GDM) and periodontal inflammation by means of periodontal pathogens [Porphyromonas gingivalis (P.g), Prevotella intermedia (P.i), Tannerella forsythia (T.f), and Treponema denticola (T.d)] and clinical periodontal parameters. 101 GDM (study group) and 98 non-GDM (control group) were included in the study. Clinical periodontal parameters were recorded, subgingival plaque samples were collected and analyzed by using qualitative polymerase chain reaction (qPCR). Clinical periodontal parameters and periodontal inflamation surface area (PISA) values were higher in GDM group. GDM participants who diagnosed with periodontitis was found to be more than non-GDM group. The number of P.gingivalis was similar in the two groups. The number of P.gingivalis increased as the PISA value increased. P.intermedia increased as the week of gestation increased. P.intermedia was found to be higher in non-GDM. The risk of GDM increased 1,208 times as the number of T.forsythia increased. T.fosythia was found to be associated with increased PISA value, periodontal diagnosis, and GDM. T.denticola was found in higher in non-GDM group. Even though it was detected in a limited group of participants, the correlation of T.denticola with GI and PISA indicates that this pathogen is related to the periodontal disease activity; not with GDM. The fluctuation of hormones and comprimised periodontal status during pregnancy accompanied by GDM resulted with a difference of the periodontopathogen numbers in the subgingival flora. It was concluded that especially T.forsythia may play a role in the association between GDM and periodontal inflammation.
