Abstract (EN)

Introduction: The use of platelet concentrates is noteworthy to accelerate the healing of extraction sockets after the extraction of impacted third molars. The aim of this study; the effects of leukocyte- and platelet-rich fibrin (L-PRF) and titanium-prepared platelet-rich fibrin (T-PRF) placed in the extraction socket after surgical extraction of impacted mandibular third molar teeth with an indication for extraction on gingival blood flow and pulpal blood flow of the adjacent second molar tooth. It is a retrospective comparison with laser doppler flowmetry. Materials and Methods: A total of 60 patients with fully impacted mesioangular mandibular third molars, aged between 20-30 years, who used L-PRF and T-PRF in surgical procedures, were selected and the changes in blood flow were evaluated retrospectively. The study was designed as 2 groups, with 30 patients selected from each group. In group 1, patients with L-PRF placed in the extraction socket and in group 2 patients with T-PRF placed in the extraction socket were examined. In each group, preoperatively and on the 7th postoperative day, the pulpal blood flow of the adjacent tooth from the same 1 spot on the surface of the adjacent second molar tooth and the gingival blood flow from 3 separate spot of mucoperiosteal flaps were measured with a Laser Doppler Flowmeter(LDF) for 30 seconds for each point. The obtained data were transferred to the computer environment and recorded by the program (perisoft for windows, perimed) and the obtained data were evaluated statistically. Results: A statistically significant increase (p<0.05) was observed in the pulpal perfusion unit(PU) of the adjacent second molar tooth and gingival perfusion unit(PU) values of the flap in both L-PRF and T-PRF groups on the postoperative 7th day, but this increase was statistically higher in the T-PRF group (p<0.05). Conclusion: Since tissue perfusion can be a good predictor for healing, T-PRF was found to be more successful during the healing period of the extraction socket.
