Comperative results of bilateral and unilateral total knee arthroplasty at patients with gonarthrosis

In this study, we retrospectively compared the results of the unilateral and bilateral total knee arthroplasty (TKA) in short, mid and long term. 102 knees of 65 patients who had enough follow up time and treated with TKA in Karadeniz Technical University Farabi Hospital, between december 1998- april 2016 were included to our study. After doing the retrospective analyzes on patient's records, detailed systematically physical exams were done. The patients were evaluated in accordance with American Knee Society Score (AKSS). X rays were evaluated in accordance with Total Knee Arthroplasty Radiographic Evaluations and Scoring System. Bilateral cases were operated simultaneously by a single team. The patients treated with unilateral and bilateral TKA were grouped short, mid and long follow up time. The patients were defined as "short term" who had 0-12 month follow up time, 12-60 month follow up," midterm" who had 0-12 month follow up time and "long term" who had over 60 month follow up time. The results were compared with literature. Kolmogorow-Smirnow, Student T test and Mann Whitney U test were used in statistical evaluation. We didn't find any significant difference in knee scores, knee functional scores, range of motion and transfusion rate at comparison of unilateral and bilateral TKA according to our study. We found one significant difference in favor of bilateral group, in comparison with unilateral and bilateral TKA short term ROM. We didn't find any significant result in comparison mid and long term ROM comparisons. Our results were evaluated as successful when it was compared with literature. As a result of these study and literature, it can be selected as a successful treatment method in terms of providing good functional results of applying bilateral TKA in selected in patients with appropriate indications. Surgical experience, carefully analyzing of the patient's medical situation and active postoperative rehabilitation are also necessary factors for successful results. In this study it was shown bilateral TKA did not increase the risk of mortality and morbidity of the patient.
