The relationship between depression, anxiety, stress levels, chronic disease self-management, and quality of life of hemodialysis patients

Abstract (EN)

The present study aimed to investigate the relationship between depression, anxiety and stress levels of hemodialysis patients and chronic disease self-management and quality of life. This descriptive and cross-sectional study was conducted in Karaman between February and October 2023 with a total of 112 hemodialysis patients who were aged 18 and over who volunteered to participate in the study. The data of the study were collected face to face with the help of Personal Data Form, Depression Anxiety Stress Scale (DASS-21), Chronic Disease Self-Management Scale (CDSMS), and Quality of Life Scale-Short Form (QoL SF-12). A total of 63.4% of the participants were male and had an average age of 59.39±14.86 (Min: 19, Max: 83). According to the Multivariate Linear Regression Analysis, among the sub-dimensions of the CDSMS, as Healthcare Effectiveness (B: 0.280, p=0.001) and Treatment Compliance (B: 0.217, p=0.016) levels increased, and among the sub-dimensions of the QoL, as Depression (B: -0.267, p=0.032) and Anxiety levels decreased (B: -0.540, p=0.000), Physical Component levels increased. No statistically significant relationships were detected between Stress, Self-Stigmatization and Coping with Stigmatization levels and Physical Component scores (p>0.05). Mental Component levels increased as the Health Care Effectiveness (B: 0.226, p=0.015) levels increased and Anxiety levels decreased (B: -0.424, p=0.000). No statistically significant relationships were detected between Depression, Stress, Self-Stigmatization, Coping with Stigmatization, and Treatment Compliance levels and Mental Component scores (p>0.05). In conclusion, psychological conditions such as depression, anxiety and stress can weaken the chronic disease self-management skills of hemodialysis patients and reduce their quality of life. Poor chronic disease self-management skills can deepen the decline in quality of life. It is of great importance to evaluate the psychosocial status of hemodialysis patients periodically and plan appropriate interventions.
