Evaluation of the effect of non-surgical periodontal treatment on clinical periodontal status with oral health impact profile (OHIP-14) in patients with hemophilia

AIM: In this study, the effect of non-surgical periodontal treatment (NSPT) on quality of life in patients with hemophilia was examined with Oral Health Impact Profile-14 (OHIP-14). Besides, by determining before and after clinical periodontal paratemeters and demografic data in hemophilia, they were compared with healthy individuals. MATERIAL AND METHODS: 71 patients with hemophilia ( Hemophilia group; Hemophilia A: 60 patients and Hemophilia B: 11 patients) and 75 healthy individuals (control group) were included to this study and they were classified according to their periodontal situations. Demografic characteristics and behavioral characteristics related to oral health of patients were analyzed. Before NSPT and after six weeks, OHIP-14 survey was applied and clinical periodontal parameters were measured. All the data obtained was analyzed statistically. RESULTS: Statistical significantly difference was not observed between the groups in terms of patients' demografic data and behavioral characteristics related to oral health before/after NSPT (except educational status) (p>0.05). Before treatment, gingival index(GI), plaque index(PI), and OHIP-14 in patients with hemophilia were higher than controls (p<0.05). While GI and PI were higher in patients with hemophilia than controls after treatment (p<0.05), OHIP-14 did not show a significantly difference (p>0.05). OHIP-14 values decreased after treatment than before treatment in both groups (p<0.05). While stronger relationships in positive aspects were observed between OHIP-14 and all the clinical periodontal parameters in patients with hemophilia than controls both before and after NSPT, the strongest relationship was between OHIP-14 and bleeding on probing (BOP). DISCUSSION: According to the findings of our study, while oral health related quality of life in patients with hemophilia was lower than healthy individuals before NSPT, it increased after NSPT. Even it rose to the healthy individual level. Additionally, our findings showing the strong relationship between OHIP-14 and BOP indicate that gingival bleeding in hemophilia affects oral health related quality of life in negative aspect.
