Abstract (EN)

In this study which has done as the internal medicine "specialization" thesis, 72 patients of chronic viral hepatitis B, C, cirrhosis and benign cholecystopathy and 18 cases with dyspeptic complaints as the control group were took part. In the recent years, H. Pylori (Hp) prevalance increase in hepatobiliary diseases ( chronic viral hepatitis, cirrhosis, malign-benign cholecystopathies) and possibility of its role especially in the inflammation and carcinogenesis matter were also investigated. Tumor markers (CEA, CA19-9, AFP) are used for follow-up of the disease, not for the purpose of diagnosis in the hepatobiliary system cancers. Their role in the chronic viral hepatitis, cirrhosis, malign-benign cholecystopathies and their place in the diagnosis are controversial. In our evaluation, even moderate increases are seen in benign cholecystopathies, CEA, CA 19-9, AFP level detection has not found significant. It's also same in chronic viral hepatitis B,C. The significant CA 19-9 increase has seen only in cirrhotic patient group. CA 19-9 level increase in cirrhotic patients has been reported. This condition has been explained as serum level increase because of increased CA 19-9 production, cholestasis and decreased clearence. CA 19-9 increase in cirrhotic patients has not been associated with a malign process. Even our patient number is limited, endoscopic Hp examination has no significant difference between the individuals with chronic viral hepatits B and C, cirrhosis, benign cholecystopathy and the control group. As a general rule, upper gastrointestinal system endoscopy should be performed in the individuals who are elderly, has dyspeptic complaint and has alerting symptoms as hemorrhage and anemia. Peptic ulcus and it's complications can be prevented with Hp detection and then. We can also decrease the possible role of Hp in carcinogenesis of hepatobiliary system.
