ABSTRACT Purpose: To evaluate CSF dynamics of IIH and hydrocephalus and compare MRI findings with CSF parametrics and CSF pressure in IIH. Materials and Methods: CSF flow study of healthy (30) subjects and subjects with IIH (29) and high-pressure hydrocephalus (43) were enrolled. At the level of AS, ASV, peak and average velocity, forward- reverse-net flow volume, AS area, PPS/AS rate, systolic and diastolic flow durations and velocities and average flow volume per one cardiac cycle were evaluated. Non-parametric Kruskal-Wallis test (p=0,05) was used to compare three groups, Mann-Whitney U test with Bonferroni correction to compare two groups (p=0,016). MRI findings of IIH were compared with CSF parametrics and CSF pressures by non-parametric Mann-Whitney U test (p=0,05). Results: ASV and peak velocities were significantly higher in hydrocephalus amongs others. In IIH comparison with normal group, ASV was nonsignificantly a little lower and standardized average diastolic duration and sum and difference of systolic and diastolic flow durations were statistically significantly lower. In hydrocephalus compared with normal group, PPS/AS and reverse/forward flow duration rates were significantly lower and ASV, peak velocity, forward-reverse-net flow volume, average flow volume per one cardiac cycle, average AS area and average systolic and diastolic flow velocities were significantly higher. In IIH compared with hydrocephalus, PPS/AS was significantly higher and ASV, peak velocity, forward-reverse-net flow volume, average flow volume per one cardiac cycle, average AS area, average systolic and diastolic flow velocities and standardized systolic and sum of systolic and diastolic flow durations were significantly lower. In IIH, no correlation between MRI findings and ASV but a positive correlation with presence of transverse sinus compression and CSF pressures was noted. Conclusion: Significant increases in ASV and peak velocities in hydrocephalus and no difference in ASV between normal and IIH groups. In IIH, CSF pressure is positively correlated with presence of transverse sinus compression and standardized average diastolic and sum and difference of systolic-diastolic flow durations were statistically significantly short.
