Comparison of the psychological conditions of patients before and after histerektomi in relation to the type of operation and the indications of operations

Abstract (EN)
Hysterectomy is a common gynecologic operation both in Turkey and in the World. Since the loss of the uterus leads to the loss of the woman's reproduction, fertility and menstruation, it can cause the woman to feel incomplete and may cause deterioration of her mental health. The aim of this study was to compare whether the depression levels measured by the Patient Health Questionnaire in women who underwent hysterectomy in a tertiary Research and Education Hospital increase after the operation compared to the preoperative level, according to different types of surgery, gravida and parity numbers. This cross-sectional study was conducted with patients who applied to the Gynecology and Obstetrics Outpatient Clinic of Adıyaman University Training and Research Hospital between 01-06-2021 and 01-12-2021 and were planned to undergo hysterectomy. Data were obtained through interviews conducted before and 7-10 days after the operation. A hundred patients who met the inclusion criteria were included in the study. The mean age of 100 patients included in the study was 56.02 ±10.03 (min: 39, max: 81). The mean gravida value of the patients was 7.00 (±2.28) and the mean parity value was 5.90 (±1.78). There was a significant difference between mean the HSA-9 scores of the patients according to the type of surgery in the preoperative period (p=0,010). The concept of posthysterectomy syndrome was introduced in 1974 to describe the post-operative mental problems experienced by women who had a hysterectomy. Multiple studies in literature revealed that mental disorders, including depression, anxiety disorders, insomnia and psychosomatic disorders, increase after hysterectomy. Consistent with the literature, in our study, the HSA-9 score was higher in the postoperative period than in the preoperative period, but the difference was not statistically significant. The results suggest that hysterectomy has significant effects on the mental state of women and patients should be evaluated for mental health deterioration both preoperatively and postoperatively.
