Abstract (EN)

Our aim of this study was to compare of urodynamic parameters at preoperative and postoperative period in patients undergoing intrafascial or extrafascial abdominal hysterectomy with benign gynaecologic indications.Material and method: Women scheduled for total abdominal hysterectomy were randomized to the extrafascial (n=27) and the intrafascial techniques (n=24) at Obstetrics and Gynecology Department of Karadeniz Technical University Faculty of Medicine from November 2010 to April 2011. The groups were controlled for demographic variables, obstetric and gynecologic history, indications for hysterectomy, and preoperative hemoglobin values. Exclusion criteria were total prolapsus, urinary incontinance associated with chronic diseases (diabetes mellitus), vertebral hernia, neurological disorders, hormone replacement therapy and malignancy. The all patients in two techniques were evaluated urodynamic investigation at preoperative and sixth postoperative weeks. Cystometry were perfomed in urodynamic investigation at preoperative and postoperative period. All of 10 urodinamic parameters investigated in two techniques preoperative and postoperative were compared. P<0.05 was chosen to represent statistical significance for every variables. Results: Mean age was 48.49 ± 4.87. Mean body mass index was 30.316 ±5.76. We observed by urodynamic investigation at preoperative and sixth postoperative days no urinary incontinance in patients intrafascial or extrafascial total abdominal hysterectomy. There is no difference between two techniques regarding with maximum detrussor pressure, maximum abdominal pressure, strong micturition requirement, vesical compliance, detrusor compliance, maximum vesical pressure, first sensation of bladder filling, first disere to void and strong desire to void (p>0.05). Maximum bladder capacity in intrafascial technique were significantly lower than extrafascial technique (p<0.05).Conclusion: We did not found significant relationship between two groups regarding with rising of stress or urge urinary incontinance symptoms and consist of de novo incontinance after hysterectomy.
