Clinical and functional results of high tibial osteotomy and unicompartmantal knee arthroplasty in patients with isolated medial gonartrosis

Abstract (EN)
Purpose: Osteoarthritis; It is a chronic and painful disease characterized by progressive destruction of cartilage tissue, osteophyte formation, and sclerosis in the subchondral bone. For this reason, many patients apply to the hospital every year due to joint pain due to this disease. In this study, it was aimed to determine the clinical and functional results of patients with high tibial osteotomy and unicompartmental knee arthroplasty performed to patients with isolated medial gonarthrosis by a single physician between 2014-2019 in our clinic. Materials and Methods: A total of 70 patients, 17 male (24.2%) and 53 female (75.8%), between the ages of 40 and 79 who underwent or HTO by a single physician between March 2014 and February 2020 were included in the study. KOOS (Knee Osteoarthritis Outcome Score) form was filled in to the operated patients during the study. During the study, the patients were also given sit-stand test and 6-minute walking tests to measure the functional status of the patients objectively. Results: In terms of all operations, the frequency of those with a good KOOS score (70-79.9 points) was f = 13 (16.88%), and the frequency of those with very good (80 - 89.9 points) was f = 29 (37%, 66). The frequency of those with excellent KOOS score (between 90 and 100 points) in terms of all operations is f = 25 (32.46%). Multivariate analysis of variance (MANOVA) was conducted among the operation 62 type (UKA vs. HTO). It was observed that the groups did not differ in terms of KOOS scores. Whether there is a difference between the type of operation (UKA vs. HTO) in terms of functional performance parameters, sit-to-stand test score and 6-minute walk score, was examined by multivariate analysis of variance (MANOVA) in R Studio compiler. Analyzes show that groups differ from each other in terms of sit and stand test score. Accordingly, the average sit and stand test score (¯X = 25.23) of the UKA-operated knees (N = 38) was significantly higher than the HTO-operated knees (N = 39, ¯X = 20.08) (F (1.75) = 7.55, p <.01). Conclusion: When HTO and UKA surgeries are performed with the right indication, the results are satisfactory. According to the data in our study, we found the patients results 87% successful (KOOS> 70). However, there was no significant difference between the groups. In functional parameters, HTO was significantly better between the two groups in the sit and stand test.
