Compare the cardiac perfusion magnetic resonance imaging (MRI) with myocardial spect in the detection of subendocardial hypoperfusion in Cardiac Syndrome X patients

There are many controversies in the diagnosis, treatment and prognosis of thecardiac syndrome X (CSX). This syndrome is no more accepted as benign as itwas accepted before. Especially, in the patients who have subendocardialischemia due to endothelial dysfunction , prognosis of CSX resembles obstructivecoronary artery disease (CAD).Objectives of this study is to compare the cardiac perfusion MagneticResonance İmaging (MRİ) with myocardial SPECT in the detection ofsubendocardial hypoperfusion in CSX patients and to measure the patients hs-CRP and NT-proBNP values and investigate whether there is a relationshipbetween the levels of these markers and 2 imaging technics or not.Between October 2011-April 2012, 20 CSX patients who were older than 18years old, and admitted to Gazi University Faculty of Medicine CardiologyDeparment were enrolled for this study. These patients underwent cardiacperfusion MRİ and SPECT studies. Also hs-CRP and NT-proBNP levels weremeasured.Of these 20 patients, 13 (65 %) showed subendocardial perfusion defect oncardiac perfusion MRİ but only 7 of these patients (35 %) showed reversibleischemia on SPECT study. Significantly more patients in pathologic MRİ findinggroup experienced chest pain that is induced by adenosine than normal findings61MRİ group (61,5 % vs 14,3 %, p=0,04). There was no association betweenpatients basal characteristics and MRİ results.We could not find any correlation between NT-proBNP measures of thepatients and imaging modalities.hs-CRP levels of the patients were higher in pathologic MRI findings groupthan normal MRİ findings group (0,4 mg/dl vs 0,11 mg/dl, p=0,03). The same wastrue for also myocardial SPECT group (0,46 mg/dl for SPECT positive vs 0,21mg/dl, for SPECT negative patients p=0,04).
