COMPARISON OF CHRONIC OBSTRUCTIVE LUNG DISEASE DIAGNOSED PATIENT WITH NOT CHRONIC OBSTRUCTIVE LUNG DISEASE DIAGNOSED PATIENT IN CARDIOPULMONARY BYPASS SURGERY ABSTRACT Objective: The aim of this study is intended to be revealed the relationship between cardiopulmonary bypass (CPB) effect and chronic obstructive pulmonary disease (COPD) patients. In this concept, the aim of this thesis was to investigate the intubation time, intensive care unit (ICU) stay, plasma lactate levels in blood, infection development status of the group of patients who have been diagnosed with COPD. Also, the study targeted to compare the patients (undergone cardiopulmonary bypass) of the group who have been diagnosed with COPD. Methods: This study was performed at the Cardiovascular Surgery Department in İzmir Tepecik Training and Research Hospital with elective open heart surgery patients who were undergone cardiopulmonary between February 2013 and June 2015. As an experimental study in which retrospective data were obtained from a group of 80 patients including an experimental group (40 patients who have been diagnosed with COPD) and a control group (40 patients who have not been diagnosed with COPD). To compare both groups to each other, the data collected from the intubation period, intensive stay duration, lactate levels in plasma, infectious cases in ICU period. Results: There were no significant differences in age, mean x-clamp time, and mean CPB duration between both experiment and control groups (p > 0.05). The mean intubation duration of experiment and control groups were 327.4 and 313.6 hours, respectively. Although the intubation time of experiment group is longer than control group there was no significant difference between them (p > 0,05). Another result is shorter ICU staying duration in favor of control group (4.2 days for experiment group and 3.9 days for control group) but yet there was no significant difference between them (p > 0,05). The plasma lactate levels in the first hour of ICU were higher in the experimental group than in the control group. (p < 0.05). When the infection rates were compared between two groups, infection rate of control group is 12.5% on the other hand infection rate of experiment group is 40%. When the patients receiving blood transfusion were compared, infection observed in the control group as 16,7% while the infection incidence of the experimental group was 100% and thus, there was a significant differences between two groups (p < 0,05). Conclusion: While there were no significant differences in terms of intubation time, ICU staying time, and lactate levels between control and experimental groups, the patients of latter who were undergone CPB were found to be more prone to infection in the ICU period, Key Words: Chronic Obstructive Pulmonary Disease, Cardiopulmonary Bypass, Plasma Lactate, Intubation Time, Infection
