Evaluation of the follow-up and treatment results of patients followed in our clinic with a diagnosis of chronic hepatitis B

Abstract (EN)
Introduction and Aim:Chronic hepatitis B (CHB) remains a significant cause of morbidity and mortality worldwide. Entecavir, Tenofovir Disoproxil Fumarate (TDF), and Tenofovir Alafenamide (TAF) are the most commonly used antiviral agents. This study aimed to evaluate the clinical and laboratory characteristics of patients diagnosed with CHB, compare them based on the antiviral treatment used (TDF, TAF, Entecavir), and determine the predictive value of selected parameters for treatment success. Materials and Methods:A total of 150 patients diagnosed with CHB who were admitted to the Gastroenterology Clinic of Adıyaman Training and Research Hospital between 01.07.2020 and 23.08.2024 were retrospectively analyzed. Demographic, clinical, and laboratory data were obtained from the hospital information system. Patients were divided into three groups based on the antiviral agent used (TDF, TAF, Entecavir). Kruskal Wallis H test, One-Way ANOVA, and post hoc tests were used for statistical comparisons. Receiver Operating Characteristic (ROC) analysis was performed to determine the parameters predictive of treatment success. Results:Of the 150 patients included in the study, 66% were male and 34% were female, with a mean age of 47.66±13.60 years. TDF was the most commonly prescribed antiviral agent (76%). The median time to HBV DNA negativity was 6.5 months in the TDF group, 9 months in the TAF group, and 12 months in the Entecavir group. Laboratory parameters significantly associated with treatment success included baseline HBV DNA (cut-off: 5,394,500 IU/mL), current platelet count (cut-off: 170,000/mm³), current ALT level (cut-off: 34.5 IU/L), and current albumin level (cut-off: 3.95 g/dL), all with statistically significant results (p<0.05). Overall treatment success was achieved in 98.7% of the patients. Conclusion:TDF was found to be more effective in achieving earlier virological response. Readily accessible laboratory parameters such as ALT, platelet count, and albumin were shown to be valuable predictors of treatment success. These findings emphasize the importance of individualized treatment strategies in CHB and support the consideration of baseline laboratory markers in guiding antiviral therapy.
