Clinical studies of hip dysplasia treatment with juvenil pubic symphysiodesis (JPS) technique in dogs

The aim of this study was to evaluate the clinical and radiological findings of JPS technique for treatment of hip dysplasia in dogs. For this purpose, a totally 12 mongrel dogs puppies (10 female and 2 male), 16 ? 20 weeks old, 12 ? 18 kg body weight were used.All animals were examined clinically. Preoperatively Ortolani?s tests were positive in 6 dogs bilaterally and in 2 dogs unilaterally, also were negative other 4 dogs. Radiographies were taken in standard VD position and distraction position using Vezzoni distractor in preoperatively and 1st 3rd and 6th months postoperatively in all dogs. Norberg angle and distraction index were measured on radiograms.Following one day fasting period, dogs were anaesthetised by 0,05 mg/kg atropine sulfate (Atropin®, Vetaş) subcutaneously, 1 mg/kg xylazine HCl (Alfazyne®, Ege-Vet) intramuscularly, 10 mg/kg dose intramuscularly ketamine hydrochloride (Alfamine®, Ege-Vet) at concentrations of %2 isoflurane (isoflurane - USP ®, Adeka).Approximately 12 cm skin incision, 2 cm longer than symphisis pubis from both cranial and caudal ends, was made exactly on the median line in female dogs and just laterally to penis in the male dogs. Subcutaneous tissue was separated with blunt dissection. Bleeding due to a. and v. pudentalis was controlled with ligation. Following the blunt dissection of the fasciae of gracilis and pectineus muscles from the pubis, symphysis pubis was exposed. During electrocauterization, protection of the rectum and urethra was accomplished by insertion a spatula into the cranial pelvic canal. To obtain necrosis of the growth plate, 2/3 parts of pubis was cauterizated by using 40 W electrocauter, starting from the proximal to the distal of the symphysis pubis. Cauterization procedure was repeated in 10 seconds intervals. After each cauterization, because of the protection of surgical area and other related tissues, region was washed with warm saline solution. The results of Ortolani?s tests postoperatively, 1 months, three dogs were bilaterally positive, the other nine dogs were negative. Also postoperatively 3 and 6 month findings of Ortolani?s method were negative.The mean and standard error of the Norberg angle measurements on the radiographs from the standard ventro?dorsal position, postoperatively 1, 3 and 6 months, 110,92 ± 1,32, 115,08 ± 1,64, 118,75 ± 1,38, 122,75 ± 0,810 for right coxofemoral joints, and 105,08 ± 2,46, 113,42 ± 1,22, 115,83 ± 0,67, 119,0 ± 0,620 for left coxofemoral joints were calculated, respectively.The mean and standard error of the distance between the centers measurements on the radiographs from the standard ventro?dorsal position, postoperatively 1, 3 and 6 months, , 0,31±0,019; 0,15±0,045; 0,066±0,035; 0,016±0,016; for right coxofemoral joints, and 0,31±0,022; 0,13±0,050; 0,058±0,031; 0,016±0,016; for left coxofemoral joints were calculated, respectively.The mean and standard error of the distance Distraction indices on the radiographs from the standard ventro?dorsal position, postoperatively 1, 3 and 6 months, , ,66 ± 0,037, 0,45 ± 0,018, 0,40 ± 0,030, 0,32 ± 0,017 cm for right coxofemoral joints, and 0,67 ± 0,059, 0,49 ± 0,039, 0,42 ± 0,021, 0,32 ± 0,013 cm for left coxofemoral joints were calculated, respectively.The mean and standard error of the distance between the centers measurements on the radiographs from the standard ventro?dorsal position, postoperatively 1, 3 and 6 months, 0,57 ± 0,038, 0,42 ± 0,016, 0,39 ± 0,028, 0,32 ± 0,017 cm, for right coxofemoral joints, and 0,58 ± 0,049, 0,44 ± 0,037, 0,39 ± 0,019, 0,32 ± 0,013 cm for left coxofemoral joints were calculated, respectively.The juvenil pubic symphysiodesis results in the increase of the Norberg angle, and decrease in both the distraction index and distance between the centers, so it was concluded that, the recommendation of the JPS for CHD prophylaxis is suitable.Key Words: Distraction of index, JPS, hip dysplasia, distance between the centers, Norberg angle
