Clinical studies on the treatment of hip dysplasia by ?modified darthroplasty? in dogs

The aim of this study was to evaluate the efficiency of ?Modified DARthroplasty? technique?s on treatment of hip dysplasia, according to the clinical and radiographical findings.For this purpose, study was carried out on 20 dogs in different breed, sexes (13 Male, 7 Female) and ages (4-24 months), which had diagnosed hip dysplasia according to clinical and radiographical findings. According to radiographical findings, DARthroplasty was performed, in 16 dogs unilaterally (11 right and 5 left), and in 4 dogs bilaterally.Following the 12 hours fasting period, anticholinergic premedication of anaesthesia was induced by subcutaneously injection of 0.045 mg/kg atropin sulfate (Atropin 20 mg/ kgEgevet ®). After that, sedative-analgesic premedication was achieved by intramuscularly injection of 1 mg\kg xylazine hydrochloride (Alfazyne 20 mg\kgEgevet® ). Ten minutes later, induction was achieved by 5 mg\kg ketamine hydrochloride (Alfamine 100mg\kgEgevet ®) injection, intravenously. Following the induction, endotracheal intubation was achieved and anaesthesia was maintained by inhalation of 2% isoflurane concentration (Isoflurane - USP, Adeka®), by using semi-closed circuits anaesthetic machine (SMS 2000 Vent?V, SMS Medical Aparts. Elec. Elecr. Build. Text. Tour. Car Ind.and Trade Ltd. Co., Ankara).Operation area is prepared according to routine asepsis and antisepsis procedure. Thirteenth rib was used as corticocancellous bone graft. According to routine surgical procedure, dorsal acetabular rim was revealed. Grafts were placed into three holes which were drilled at equal distance to each other. Area was closed in routine manner.At the postoperatively first day, it was observed that all the dogs were ambulant and lively.According to long-term follow up (10th days, 3th and 12th months) clinical tests findings obtained by abduction-external rotation test, hip extension test, standing test and walking-running test, were better when compare preoperative findings.In this study, mean Norberg Angle was measured as 92,32±2,04, 94,0±1,94 and 95,36±1,95, preoperative, postoperative 3th and 12th months respectively.As a result, in this technique ?Modified DARthroplasty?, transplanted cortico-cancellous grafts neither extend dorsal acetabular rim nor stoped degenerative changes. However, although we carried out this study in a small number of patients, we thougt that in cases which has minimal degenerative changes, problems could be solve by using this method without using prosthesis or similar devices. It was also concluded that, detailed controlled trials might be necessary in an attemp to highlight that this technique caused clinicaly recovery by denervation of joint capsule or by the effects of corticocancellous bone grafts may acting as ?stem cells?.
