Abstract (EN)
In the present thesis, the effects of isoxuprine hydrochloride (IP) administered before planned and emergency cesarean sections on mother and newborn were investigated. Maternal and fetal circulation by Doppler ultrasonography, maternal effects by oxidative stress and blood gas analysis, and neonatal survival by modified APGAR score and blood gas analysis were investigated. The study was conducted on 40 pregnant 2-year-old Ivesi ewes, and 2 different operation groups were formed as planned cesarean section and emergency cesarean section. A total of four groups were formed from these groups. Group I (n=10): Intramuscular 5 mL isotonic 45 minutes before cesarean section, Group II (n=10): 0.6 mg/kg IP 45 minutes before cesarean section, Group III (n=10): 16 mg dexamethasone 24 hours before cesarean section, Group IV (n=10): Ewes treated with 16 mg dexamethasone 24 hours before cesarean section and 0.6 mg/kg IP 45 minutes before cesarean section. Before cesarean section, Doppler measurements were performed on uterine artery, umbilical artery and fetal aorta; blood gas and oxidative stress parameters were evaluated preop and postop. PI and RI values decreased in isoxuprine-treated groups, whereas PSV, EDV and A values increased (p<0.001). In blood gas parameters, pH, O2 ve HCO3values were higher and pCO2 values were lower in IP-treated groups. APGAR scores were also higher in the groups given IP. In conclusion, IP administration improves maternal and fetal circulation, increases blood flow and has a favorable effect on newborn prognosis. In addition, it shows the positive effects of isoxuprine use before cesarean section on maternal and fetal health, but additional studies comparing different doses, duration of administration and alternative tocolytic agents are needed.
