Evaluatıon Of Embryo Development And Pregnancy In Ivf Patıents Wıth Apoptosıs And Luteınızatıon Process In Cumulus Cell Culture The rapid formation of the cumulus matrix around the oocyte in the pre-ovulation period is a unique and dynamic biological event. Granulosa cells surrounding the oocyte form a cumulus oophorus. Cumulus cells play an important role in ensuring adequate nutrition and growth capacity of reproductive cells. Cumulus, granulosa and teka cells around oocytes contribute to oocyte maturation by steroid synthesis. Apoptosis occurs in the normal process of cumulus granulosa cells and cumulus apoptosis is associated with oocyte maturation. There are also studies that early luteinization deteriorates oocyte quality with progesterone secretion and negatively affects pregnancy. Studies have shown that high apoptosis negatively affects pregnancy in these cells. In this study, it was aimed to investigate the progression of cumulus cells left in culture to apoptosis and progesterone secretion, which is a sign of luteinization, the relationship between embryo development, morphology and pregnancy. This study was carried out with the culture of cumulus granulosa cells taken from 40 IVF women without any indication who applied to Bahçelievler Medical Park Hospital IVF Clinic between September 2017 and April 2018. For this purpose, the study was submitted to the uni 'Biruni University Non-Interventional Ethics Committee' ve and approval of the Ethics Committee dated 26.04.2017 and numbered 2017 / 5-8. Cumulus granulosa cell samples were taken after application of hyaluronidase, washed twice in HTF solution and cells were seeded at a concentration of 5x105 / ml, followed by 24-hour, 48-hour, 72-hour, 96-hour periods, and apoptotic cell rates were measured and serum progesterone measurements were performed on HCG day. In this study, it was shown that cumulus cell apoptosis was lower in the pregnant group compared to the non-pregnant group (p = 0.023), and progesterone value on HCG day was higher in non-pregnant group (p = 0.021). From the data obtained until the 4th day of culture, it was understood that apoptosis rate showed a higher tendency in non-pregnant women and this was statistically significant (p = 0.009). In our study, it was found that the ratio of apoptotic cells up to the 4th day showed a positive correlation with the progesterone value on hCG day (p = 0.001). In this study, it was understood that cumulus cell apoptosis and hCG day progesterone levels were associated with clinical pregnancy and embryo quality.
