Comparing three different drugs with the postoperative patient controlled analgesia method in the case of lomber disc hernia

COMPARİNG THREE DIFFERENT DRUGS WITH THE POSTOPERATIVEPATIENT CONTROLLED ANALGESİA METHODIN THE CASE OF LOMBER DISC HERNIAZEKİ TUNCEL TEKGÜLIn this research we studied the cases scheduled for an elective lumbar disc surgerywhich are predicted to have moderate or severe pain in the postoperative period. We aimed tocompare intravenous lornoxicam PCA with morphine and meperidine in aspect of theiranalgesic potencies and adverse effects. 60 patients were separated into 3 randomized groups.Settings were for Group I; lornoxicam 8 mg loading dose, 0,3 mg/hour infusion rate, 1 mgbolus, 15 min. lock out time, 4 hours limit 8 mg, daily total dose 24 mg, for Group II;morphine 4 mg loading dose, 0,3 mg/hour infusion rate, 1mg bolus, 15 min. lock out time, 4hours limit 10 mg, daily total dose 20 mg, for Group III; 20 mg meperidine loading dose, 3mg/hour infusion rate, 10 mg bolus, 15 min lock out time, 4 hours limit 100mg, daily totaldose 200 mg.Postoperative in the recovery room, in the 1, 15, 30, 45. minutes and 1, 2, 3, 4, 6, 8,10, 12, 16, 20, 24. hours mean arterial pressures, heart rates, respiratory rates, VAS, sedationscores, nausea/vomiting, pruritus, distention and urinary retention levels were recorded.In our study, VAS, pain intensity difference, sum pain intensity difference and overallassessment of pain scorings were used to evaluate pain. In all groups VAS scores, painintensity differences, sum pain intensity differences and overall assessment of pain scale datawere similar. When the adverse effects are compared, the frequency of the advers events oflornoxicam seemed very less and it is more tolarable by the patients.We conclude that in the control of moderate and severe postoperative pain, lornoxicamPCA provides effective analgesia similar as opioids with less adverse effects respectively.
