Early surgery results in patients with perianal fistula observed by magnetic resonance imaging pre surgery

Abstract (EN)

Aim: Perianal fistulas are an important cause of morbidity. If the appropriate surgical technique is not used, the disease may reccur. In this study, we aimed to determine the early results of fıstulotomy and seton surgery in anal fistula patients and to find the diagnostic value of MRI by comparing surgical findings with MRI. Matherial and methods: In this study, 145 patients operated for perianal fistula in K.T.U. Faculty of Medicine Farabi Hastanesi General Surgery Clinic between January 2015 and January 2020. All patients were imaged with Lower abdomen MRI before surgery. Lower abdomen MRI was used in the follow-up of the patients. Our surgical techniques were fistulotomy and seton surgeries. The followed-up was done for at least 5 months. Findings: A total of 111 patients were included in the study. 87 of the patients (78%) were male and 24 (22%) were female. The age distribution of the cases was between 19-83 and the mean age was 45.06. According to Parks classification in this study; 70 patients (63%) had intersphincteric, 33 patients (%29,7) had transsphincteric, and 8 patients (6%) had suprasphincteric fistulae. No extrasphincteric fistula was found in ourstudy. Results: Consequently, it is necessary to determine the type of fistula by MRI before surgery. Patients should be carefully informed about possible incontinence and recurrence complications. The limitation of this study is that it was conducted in a single center and reflects short term results.
