Abstract (EN)
Gastric carcinoma is second common cause of death due to cancer worldwide. Incidence is decreasing in developed countries, although occurence rates of proximal gastric cancers are rising with bad prognostic properties, causes of this process is still unexplained.In our study, tumoral characteristics of 366 patients who has been operated for gastric cancer in our clinic between 1990-1999 (Period A), and 2000-2009 (Period B) is assessed and comparison of data between two periods is done.Age, tumor localization, depth of gastric cancer invasion, histological type, lymph node invasion status of operated patients was examined in two periods, both, and comparisons between periods has been made.In statistical assessments; In Period B; an increased incidence of proximal gastric cancers (PGC) due to up-rising tumors which sighted in cardiac localization, an increased number of patients who went directly to surgery in stage T2 of Mid-Distal Gastric Carcinoma (M-DGC), and an increased rate of total gastrectomy surgery is determined significantly; In Period A, stage T3 in M-DGC was seen more than PGC?s, and in last two decades; operated patients were principally in stage T3, stage T4 tumors were seen much more in PGC?s, lymph node invasion rates were significantly higher in PGC?sPGC?s are more common in the last decade, tumoral lymph node invasion rates are increased in PGC, and more advanced surgery has been applied. Primary aim in gastric carcinoma treatment is detecting the tumor in early stages, while stage T3, which has a poor prognostic value, and higher tumoral surgery has been applied more commonly in our clinic, and surgery rates of patients with early stage remained very low.
