The relationship between ventricular arythmies and electrocardiographic repolarization parameters in mitral valve prolapsus syndrome

Abstract (EN)

Objectives: Mitral Kapake prolapse (MKP) is one of the common cardiac abnormalities. The incidence of infective endocarditis, cerebral embolism and arrhythmia is increased in people with this Kapake pathology. Ventricular and supraventricular arrhythmias may occur. Although the risk of sudden cardiac death is low, it is increased compared to the normal population. Mitral Kapake prolapse is more common in patients with severe mitral regurgitation. In addition, an increase in electrocardiographic repolarization disorders was reported in MKP. In our study we aimed to investigate the incidence of ventricular arrhythmias and the relationship between ventricular arrhythmias and repolarization parameters in patients with mitral Kapake prolapse syndrome. Material and Methods: A total of 120 patients (46 females, 36 males) were included in this study. We compared 41 patients with palpitations who were diagnosed with MKP syndrome and 41 subjects with palpitations had normal echocardiographic findings. For this purpose, 24-hours rhythm holter and electrocardiography data were compared. SPSS 16.0.0 program (IBM) was used for statistical analysis. For comparison of continuous data between groups; mean, standard deviation, independent student'T test was used for regularly distributed data and Mann Whitney U test was used for non-regularly distributed data. Chi-square test was used to analyze categorical data and differences between groups. For normal correlation analysis, Sperman and Pearson tests were performed for correlation analysis. P values less than 0.05 were accepted for statistical significance. Results: In this study, the incidence of ventricular premature beats and non sustained ventricular tachycardia were found to be higher in the group with MKP syndrome than in the control group (p values = <0.001, 0.012, respectively). Although the frequency of supraventricular premature beats and atrial fibrillation were more common in the group with MKP syndrome, it was not statistically significant (p = 0.064, p = 0.056, respectively). There was no statistically significant difference between the two groups in terms of QTc values among the repolarization parameters (p = 0.056). T peak-end interval was higher in MKP group (p = 0.001). As the diameter of the left atrium and the degree of mitral regurgitation increased, the frequency of ventricular premature beats increased (r = 0.394, p <0.001, r = 0.517, p <0.001, respectively). Conclusion: It was thought that the frequency of ventricular arrhythmias increased in individuals with mitral Kapake prolapse syndrome, and the frequency of these arrhythmia was directly proportional to the left atrial diameter and the degree of mitral regurgitation and T peak-end interval measurement could be useful in predicting these arrhythmias.
