9SUMMARYThe aim of this study is to determine the accordance of charging patients inneurosurgery clinic between the physician?s request form and nurse follow-up form.The study materials were chosen from the charging patients in neurosurgery clinicbetween February 2004 and December 2005. We evaluated the file of 50 patientsfrom neurosurgery clinic archive.We prepared a form which include the name, surname, age, gender, address,diagnosis, duration of charge, protocole number, and department of the chosenpatients.We compared in our form the accordance between the physician?s follow-up form(include the vital signs: fever, arterial pulse, arterial blood pressure, extremity-consciousness-pupilla follow-up, parenteral and oral drug therapies follow-up) andnurse follow-up form in the first week of patient charging.We evaluated the datas with percent and aritmetic mean numbers.The results of study decleared that fever follow-up 13.90%, arterial pulse, arterialblood pressure and breathing follow-up 11,92% had not been applied according tothe physician?s request form.Parenteral therapy follow-up 6.94%, follow-up of liquid that the patient took inand took out orally and parenterally 24.61%, and extremity-consciousness-pupillafollow-up 40% had not been applied according to the physician?s request form.Oral drugs 4.18%, parenteral drugs 2.88%, had not been applied according to thephysician?s request form.Key words: Physician?s request form, nurse follow-up formPDF created with pdfFactory trial version www.pdffactory.com
