Investigation of the results of Lindgren-Turan method in the surgical treatment of patients with moderate and severe hallux valgus

Abstract (EN)
Aim: The aim of this study was to investigate the radiologic, clinical and cosmetic results of surgical treatment of patients with moderate to severe hallux valgus using the Lindgren-Turan method. Method: Between 01.01.2017-01.06.2023, 63 patients out of 106 patients who applied to the outpatient clinic between 01.01.2017-01.06.2023 with complaints of foot pain and inability to use shoes, and who were operated by Lindgren-Turan osteotomy for the deformity as a result of hallux valgus as a result of physical examination and examinations were included in the study. The ages of the patients included in the study were between 18-60 years. Patients who did not meet this age range were excluded from the study. Patients with arthrosis or deformity of the 1st metatarsophalangeal (MP) joint due to a rheumatologic disease, patients who used a surgical technique other than the Lindgren-Turan method or a technique with additional osteotomy were not included in the study. It was ensured that all patients were operated with the same surgical technique. Patients were evaluated at regular follow-up visits for at least 12 months. Gender differences of the patients were not considered in the study. Radiologic examinations, AOFAS score and cosmetic satisfaction of the patients included in the study were investigated. Results: When the postoperative AOFAS scores, Hallux Valgus Angle (HVA), Intermetatarsal Angle (IMA) and cosmetic satisfaction of 63 patients included in the study were evaluated, it was seen that the hallux valgus angle was moderate in 53 patients and advanced in 10 patients in the preoperative period, while the hallux valgus angle was mild in 51 patients and moderate in 12 patients in the postoperative period. The intermetatarsal angle values were mild in 2 patients, moderate in 49 patients and severe in 12 patients preoperatively, and mild in 45 patients and moderate in 18 patients postoperatively. When AOFAS scores were analyzed, it was noted that 51 patients scored above 80 points and 44 of these patients were cosmetically satisfied. 12 patients scored below 80 points and 8 of them were not cosmetically satisfied. Of these patients, 6 had advanced hallux valgus angle and 6 had intermediate hallux valgus angle. Among the cosmetically dissatisfied patients, 4 had intermediate hallux valgus angle and 4 had advanced hallux valgus angle. The amount of change in hallux valgus angles was found to be associated with AOFAS score and cosmetic satisfaction. When the effect of the change in intermetatarsal angle values in terms of AOFAS score and cosmetic satisfaction was evaluated, no significant correlation was found. In the postoperative follow-up of 2 patients with hallux valgus values of 52 and 48, it was observed that the hallux valgus value increased again and the deformity occurred again. 1 patient developed soft tissue infection due to implant irritation at the 6th postoperative month. Conclusion: The use of the Lindgren-Turan surgical method in patients with moderate hallux valgus has been found to give very good results in terms of patient satisfaction and cosmetic results. Although this method gives less satisfactory results in patients with advanced hallux valgus compared to patients with moderate hallux valgus cosmetically and according to AOFAS scoring, it has been shown to be a successful surgical method in patients with advanced deformity.
