Abstract (EN)

This study aims to compare the cortical and spongious bone structures of patients with low-energy trauma and hip fracture despite osteoporotic bone mineral density (BMD) values, and of patients with low-energy trauma and hip fracture despite osteopenia values, in the light of the micro-architecture and cellular of the bone structures, and thus to evaluate the results of this study together with the N- telopeptid (NTx) results.14 patients having visited our clinics between the dates of December 2009 and May 2011 are included in the study. By means of their BMD values, these patients are divided into two groups: 7 with osteoporotic values and 7 with osteopenia values. Then, the urine samples are taken from these patients and their NTx values are checked. After the required examinations are completed, these patients are taken for the operation to have partial endo-prosthesis, and during the operation, some samples of cortical bone and spongious bone are taken from the fractured hip. These samples are examined by means of scanning electron microscopy (SEM) and transmission electron microscopy (TEM).As a result of these examinations, it is founded out that there is not a significant difference in the spongious bone between the osteoporotic and osteopenia groups, and that the disease leads to the dilution and fractions of trabecula in both groups. Also, in the cortical bone, though there is a significant difference in the cortical thickness in the osteopenia group, cells and micro-architecture depict similar features.Within the NTx results, there is again no significant difference, yet it is predicted that the destruction level is high in this period in both groups.All these results show us that BMD is not sufficient itself in order to evaluate the osteoporosis.
