Long-term results of anterior cruciate ligament reconstructions with Peroneus longus tendon graft

Objective: The aim of this study is to evaluate the long-term results of patients who underwent anterior cruciate ligament reconstruction (ACLR) with peroneus longus tendon (PLT). Methods: 26 patients who underwent ACLR with PLT graft between 2000 and 2011 and were followed for an average of 19.5 years included in the study. The average age was 45. Functional scores of the knees, laxity status (lachman-pivot shift), knee and ankle joint range of motion (ROM), thigh and cruris diameter measurements, osteoarthritis (OA) status in the knee and ankle and ankle functional scores were examined. At the same time, measurements were made to evaluate the deformity and additional pathologies that may occur in the ankle in the long term due to PLT graft harvesting. All these parameters were compared with the healthy side. Results: There was a significant increase in Lachman test grade in the operated knee compared to the healthy knee. When knee flexions were compared, it was seen that there was a significant limitation in the operated knee compared to the healthy knee. The diameter of the thigh and cruris on the operated side was significantly reduced compared to the healthy side. Postoperative IKDC average (International Knee Documentation Committee) was 82 and Lysholm score average was 88. There was a significant increase in tibiofemoral OA in the operated knee compared to the healthy knee. There was no significant difference between the operated and healthy side in terms of tibiotalar OA. No significant difference was seen for all angles measured in the operated and healthy feet in the x-rays taken in terms of deformity and additional pathology. Conclusion: The results in the operated knee after ACLR with PLT graft were not as successful as in the healthy knee. However, when the foot from which the PLT graft was taken is compared to the other foot, long-term results are promising.
