The relationship between mean platelet volume and renal ultrasonographic findings in pre-dialysis patients

Abstract (EN)
It's known that subclinical inflammation is presented in chronic renal failure. Although there are various studies on inflammatory markers in these patients it has been reported in recent years that MPV can also be used as an indicator of inflammation in different inflammatory diseases. Studies have also published that MPV level may change in both pre-dialysis and dialysis patients. However, there are not enough studies on the relationship between CKD's renal ultrasonographic findings at different stages and MPV. In this study we aimed to evaluate the relationship between MPV and renal ultrasonographic findings in pre-dialysis patients. Methods The files of pre-dialysis patients has been retrospectively examined in Department of Nephrology, Evliya Celebi Training and Research Hospital, Medical Faculty of Dumlupinar University whose GFR value less than 60ml/min/1.73m2 for at least 3 months. Patients with CKD stage 3 (n=44) and stage 4 (n=42) and who are not dialysed yet included in the study. Demographic characteristics of the patients ( age, gender ), additional diseases, complete blood count, inflammatory and biochemical parameters ( hemoglobin, wbc, neutrophils, pdw, rdw, crp, bun, urea, creatinine, sodium, potassium, calcium, phosphorus, uric acid, albumin, parathormone) renal ultrasonographic findings ( prenchymal echogenecity, parenchymal thickness, kidney size ) were obtained retrospectively from the archives of Nephrology Department and Evliya Celebi Training and Research Hospital. In this study the datas were analyzed with Statistical Package for Social Science ( SPSS ) 18.0 package program. Results When the relationship between MPV and renal ultrasonographic findings were examined, no significant correlation was found between MPV and cortical thickness ( p=0.56, r=-0.06 )and renal dimension. ( p=0.84, r=-0.02 ) Also there was no significant relationship between MPV and renal parenchymal echogenecity. There was no statistically significant correlation between MPV and neutrophil, neutrophil/lymphocytes ratio, wbc, rdw, creatinine, albumin, uric acid, pH, HCO3. ( p>0.08 ) Correlation between MPV and GFR, CRP, sediments, calcium, phosphorus, sodium, potassium, ferritin, parathormone was negative, but it was not statistically significant. ( p> 0.05 ) There was negative correlation between MPV and renal size ( p=0.23, r=-0.17 ) and MPV and cortical thickness ( p=0.20, r=-0.18 ) when we analyzed the relationship between MPV and ultrasonographic findings in patients with diabetes. But no statical significance was found. In diabetic patients, the significant correlation was found only between MPV and renal parenchymal echogenicity. ( p= 0.04 ) Conclusion In conclusion, while there was a statistically insignificant relationship between MPV and renal ultrasonographic findings in pre-dialysis patients with CKD, there was a significant relationship between MPV and renal parenchymal echogenicity in diabetic patients.
