Retrospective comparison of perioperative findings of patients with preoperative 24 hour urine diagnostic tests positive and negative pheochromocytoma

Surgical treatment of pheochromocytoma always concerns both the anesthesiologist and the surgeon. Pre-treatment measures can be taken and preparations can be appliedto the patients who are symptomatic or who have been diagnosed with pheochromocytoma in the preoperative period by laboratory or imaging methods for the hemodynamic imbalances that may develop during the operation. However, it should not be forgotten that we may encounter patients with pheochromocytoma which cannot be diagnosed in the preoperative period with diagnostic test and imaging methods. In our study, we investigated the perioperative findings, intraoperative hemodynamic imbalance treatment methods and surgical complications of patients who could not be diagnosed with pheochromocytoma with preoperative laboratory tests and imaging methods. We compared data of patients with pheochromocytoma who were diagnosedpreoperatively and perioperatively. We found that none of the patients with pheochromocytoma who had negative diagnostic tests had no impaired glucose tolerance before surgery. While all patients with a diagnosis test positive pheochromocytoma had a hypertensive crisis during surgery, some patients with negative diagnostic tests did not have hypertensive crisis during surgery. Less antihypertensive agent was used than the patients who had positive diagnostic tests in the treatment of hemodynamic instability during the operation. It was concluded that surgical resection with other antihypertensive agents could be used safely without using phentolamine during surgery.
