The evaluation of postoperative medial ligament balance for patients who undergoing total knee arthroplasty.

Purpose: In this study, especially in the knees with advanced varus deformity without using tensiometry and gap measure in the loosening of medial collateral ligament, it is aimed that the functional and biomechanical results to be assessed in the light of objective data. Material Method: 14 women and 8 male patients were included to the study for undergoing total knee arthroplasty. Each patient's radiographs of lower extremite telemetrics and valgus stress have been taken one day before the operation and 1.5 months after the operation of the knees that to be operated. HSS scores were filled simultaneously. Yet, at the same time an analysis of patients' walking have been made. The obtained data were statistically analyzed and tried to determine the differences. Patients were divided into groups according to the telemetric lower extremite and stress radiographs. Differences between the groups were investigated. Results : In the patients' HSS scores, coronal, axial and swing phase in the aligment of the ankle was observed with significant improvement. The changes in the sagittal plan, stance phase and in the alignment of ankle data were not statistically significant. (P> 0.05) The data of walking analysis compared with the control group, out of sagittal plan data of the postoperative results showed the average catch. Of the patients grouped according to the lower extremite radiographs telemetric data, the ones with the advanced varus deformity were found to be significant improvement in the postoperative rotational. The ones with the tight knees of the patients grouped according to the valgus stress radiograph data, changes in the coronal plan have been seen more obvious. Yet again, we have observed that the rotational improvement in the tight knees was not made effective as in the loose knees. Improvement in the group with the advanced coronal plan was seen to be less. As the preoperative rotational deformity progresses, the results of postoperative ankle alignment were identified to be worst. Almost in all of the temporospatial parameters that were derived from the walking analysis were seen improvement. However, the most significant changes stance percentage was observed in the double support phase and single-support phase. With the stance percentage, the reduction of the double support phase duration and the increase of the single support phase duration, yet again in the postoperative 6 week period showed normalization of the walking parameters of the patients according to the preoperational data.
