İntroductionVentilation of a single lung using double lümen intrabronchial tups is a frequently use tecnic pulmonary and thorasic surgery. Single lung ventilation may cause arterial hipoxemia and instability in the ventilation perfussion ratio due to the increase of amount of shunt in the upper lung which cannot be ventilated during surgery. After prolonged ischemia tissue reperfussion and the rapid enterance of oxygen into ischemic tissues cause production of free oxygen radicals in cells. Free oxygen radicals may lead to the production of lipid peroxidation products such as malondialdehit (MDA). This study was undertaken to show the effect of propofol and desflurane on oxidative stress and ischemia in patient with sigle lung ventilation with enzyme levels such as superokside dsmutase (SOD), glutation s transferase (GST) and catalase (KAT) and the peroxidation product MDA. The effect on ischemia was demonstrated with the ischemia modified albümin (İMA).Material and MethodForty patient ages 19-55 and ASA ?-?? rısk group, undergoing elective pulmonary surgery were included in the study the propofol group was started the infusion rate was adjusted according to the hemodinamic status. Anaesthesia was continued with %6 in the desflurane group. Analgesia in the intraoperative and postoperative phase was maintared with a thoracal epidural catater with bupivakain infussion in both groups. Blood samples were gathered in the 0., 5., 30. Minutes and 4. hour of single lung ventilation and then centrifuged and stored in -80°C for SOD, KAT, GST, MDA and IMA enzyme level measurements.ResultsWhen the IMA levels were investigeted between groups according to time, the 4. hour levels were found to be significantly lower according to the control levels in group P (p?0.05). the MDA enzyme activity were found to be simlar between both groups and within each group. SOD was found to be significantly lower ın the 4.hour levels in group D (p?0.05). GST was found to be significantly elevated in the 5. minutes level in group D (p?0.05). KAT was found to be significantly lower ın the 5. minutes and 4.hour levels in group P (p?0.05). In group D, only the 4. hour KAT levels were found to be significantly lower than the control KAT level.ConclusionAccording to this study, we can condude that desflurane increased antioxidant enzyme that SOD, GST levels and decreases KAT and oxidative stress, while propofol has benefical effects in reducing ischemia but not on antioxidant enzymes whiıch SOD, GST but only decreases KAT. To verify our data suggest that further study is needed.
