Investigation of the role of multiparametric MRI in local staging of prostate cancer

Abstract (EN)

INTRODUCTION: In recent years, the use of multiparametric magnetic resonance imaging (Mp MRI) has become an invaluable tool for anatomical and functional evaluation of prostate lesions. In addition to lesion characterization, MpMRG has been becoming increasingly used local staging of patients with cancer. The aim of this study was to determine the contribution of MP prostate MRI to the detection, localization and local staging of the index lesions MATERIALS AND METHODS: Patients who underwent total prostatectomy for prostate cancer who had MpMRI before surgery between May 2016-February 2019 were retrospectively evaluated. The pathology reports of the index lesions in the pathology specimen were retrospectively compared with the MpMRI findings using virtual mapping method. Radiologic PIRADS scores and gleason scores in pathology specimen were compared and accuracy of MpMRI for detecting clinically significant cancer was evaluated. Extracapsular invasion, extraprostatic perineural invasion and seminal vesicle invasion were noted on Mp MRI and were compared with pathology reports. RESULTS: MRI sensitivity and specifity in detection of clinically significant can cancer were 92.59% and 12,50% respectively. There was no inconsistency between MRI and pathology results in terms of extra prostatic invasion (EPI) (p> 0.05). MRI sensitivity was 64.28%, specificity was 95.24%, positive predictive value was 90% and negative predictive value was 80%. The accuracy rate was found to be 82,86%. There was complete agreement between MRI and pathology in terms of seminal vesicle invasion (SVI) (p> 0.05). There was no inconsistency between pathology and MRI in terms of extraprostatic perineural invasion (p> 0.05). Extraprostatic perineural invasion MRI sensitivity was 75%; the specificity was 100%. The positive predictive value was 100%, the negative predictive value was 93.10% and the accuracy was 94.29%. CONCLUSION: MpMRG is a valid tool in local staging and localization of prostate lesions.
