The effect of monoclonal anti tnf antibody administration in rats in injury of experimental intestinal ischemia reperfusion

Abstract (EN)
Aim: To study the effects of infliximab application, which is a monoclonal anti TNF anticor, in experimental intestinal ischemia reperfusion injury in rats.Material and Method: In this study, 48 male Wistar-Albino type rats whose weights ranged between 170 and 270 gr were used. The rats were equally divided into 6 groups, as the control group (Group I, n=8), laparotomy+infliximab (Group II, n=8), 30 minutes ischemia+30 minutes reperfusion (Group III, n=8), infliximab+ 30 minutes ischemia+ 30 minutes reperfusion (Group IV,n=8), 60 minutes ischemia+30 minutes reperfusion (Group V, n=8), and infliximab+ 60 minutes ischemia+30 minutes reperfusion (Group VI, n=8). Treatment groups were done ischemia reperfusion for planned periods of times and laparotomy, and were injected infliximab (10 mg/kg) intraperitoneally. The control group was given 3 cc %0.9 NaCl intraperitoneally. After the operation, the rats were sacrificed and blood and tissue samples were taken. The blood and tissue samples were examined by parameters such as tumor necrosis factor alfa, glutathione, glutathione peroksidas, glutathione reductose, katalas, superoxide dismutase, myeloperoksidaz, malondialdehit, and nitric oxide. In addition to these, the tissue samples were examined by light microscope being taken routine tissue follow up sections after %4 formalin fixation.Results: Following the defined I/R periods, it was understood that there were no significant differences among groups for GR, GSH, and MDA results (p=0.0952). Group III ( 30 minutes ischemia+ 30 minutes reperfusion) TNF-? values were only higher than the control group (p=0.0079). For eritrosid GSH-Px values, the values of the control group and Group II (laparotomy + infliximab) were higher than the other groups?. Similarly, the values of Group III were higher than Group VI (60 minutes ischemia+30 minutes reperfusion + infliximab) values (p=0.0006). For Eritroside SOD, the values of Group III and Group IV (30 minutes ischemia+ 30 minutes reperfusion +infliximab) were lower when compared to the values of other groups. When it came to Eritrosid CAT values, the readings of Group II were higher than Group III, Group IV, Group V (60 minutes ischemia+30 minutes reperfusion) and Group VI values. The values of Group III were lower than the control group (p<0.0001). For serum NO, Group IV values were higher than the control group, Group II, Group V, and Group VI; however, there was not a significant difference with Group III. Even so, Group III values were found high in comparison with Group II values (p<0.0001). There were not significant differences among groups for tissue TNF-? values (p=0.064). In tissue GR values, Group IV was higher than Group I, II, and V (p=0.0015). For tissue SOD values, Group III was lower than the Control group (p=0.0813). As for tissue GSH-Px values, while Group IV was higher than Groups I, II, III, and VI, there was no difference with Group V( p=0.021). Group III tissue GSH values were lower according to Group II (p=0.0329). While Group IV tissue CAT values were higher than groups I, II,V and VI, they were similar to Group III (p=0.0003). In tissue NO values, Group IV was higher than Groups I, II, and III (p=0.0073). When tissue MDA groups were examined, it was seen that Group II values were higher than Groups IV and VI (p=0.0064), and for MPO, Group III values were higher than Group V (p=0.0065).Result: We can see that infliximab has a positive anti inflammatory and antioxidant effect in the early period on intestinal I/R injury. However, more detailed research with longer ischemia and reperfusion period is needed for the determination of the possible new clinical role of infliximab and for the understanding of the later period systematic effects of it.
