Evaluation of the effectiveness of different treatment approaches in preventing coronal discoloration caused by regenerative endodontic treatment

Abstract (EN)

In this study, the effectiveness of Teethmate hypersensitivity agent, dentin bonding agent (DBA), Nd:YAG laser and ER:YAG laser in preventing discoloration caused by regenerative endodontic treatment were investigated. 105 upper incisors with a single root and single canal were included in the study. The apical portion of each specimen was resected to obtain a standardized root length of 10±1 mm below the cementoenamel junction. Root canals were prepared by ProTaper Next (Dentsply Maillefer, Ballaigues, Switzerland) X1, X2, X3, X4 and X5 using instruments up to a size of 50/0,06. In order to simulate the immature root apex, the root canals were standardized with Gates Glidden (# 2–4) drills with apical diameters of 1.1±0,1 mm. Teeth were randomly divided into 7 groups as DBA, Teethmate, Nd:YAG laser, Er:YAG laser, Biodentin, Blood, Negative control groups (n=15). Relevant dentinal tubule occlusion methods were applied to the DBA, Teethmate, Nd: YAG, Er: YAG groups. Spongostan (Cutanplast, Milano, Italy) was placed on the blood clot after the root canals were filled with blood 4 mm from the cementoenamel junction. 3 mm thickness of Biodentin (Septodont, Saint-Maur-des-Fosses, France) was placed so that it was positioned at cementoenamel junction. After restoration with composite, the teeth were stored in an incubator at 37°C and 100% humidity. Color measurement was evaluated with the spectrophotometer Vita Easyshade Advance (VITA Zahnfabrik, Bad Sackingen, Germany) before the treatment, immediately after the treatment, on the 7th, 30th, and 90th days. The data were converted to color values of Commission International de I'Eclairage L * a * b (CIELab) and ΔE values were calculated. The clinical detectability threshold of the color change was accepted as 3,3 units. The data obtained were analyzed using two-way analysis of variance and Tukey post-hoc test (p=0,05). Clinically perceptible color change was observed in all groups except the negative control group (ΔE˃3,3). However, there was no significant difference between dentinal tubule occlusion methods in preventing discoloration (p>0,05). It was found that any dentinal tubule occlusion method could prevent 100% discoloration caused by regenerative endodontic treatment.
